Please help yourself to the pdf forms below to gather your order information. Anyone can use these forms to place a bulk order—they are not exclusive to churches.
Bulk orders will be invoiced in two steps—first for the total order, and then a second time for freight costs when the order ships. Freight cost will be determined based on the quantities submitted on this form.
